Forms
For a more fluid and time-saving visit to our office, we recommend you download and fill out our New Patient paperwork prior to your visit and bring it with you. We find this helps our small patients as it leaves you time to spend with your child answering their questions or observing what they may be experiencing prior to the beginning of the appointment.
If you don’t have time to fill out the paperwork, that’s no trouble at all. We just ask that you arrive 30 minutes prior to your appointment time to fill out any necessary forms.

At Dayton Pediatric Dentistry, we do our best to provide you with options when you’re paying for your child’s dental care. We accept all major credit cards as well as cash, check, debit card, Medicaid and insurance.
